Wellness Program

The Four Seasons Wellness Program is designed to support preventive care, encourage healthy habits, and reward associates and covered spouses for taking proactive steps toward their health.

By participating, you can earn medical premium discounts, gift cards, and Wellness PTO through simple annual wellness actions.

How It Works

Associates and covered spouses enrolled in a medical plan can earn a $30 medical premium discount per person, per paycheck by completing two required steps each year:
 

• Annual preventive wellness exam

• Preventive lab work or biometric screening
   

Once those steps are completed, and submitted the verification form, you can earn additional incentives by participating in wellness activities such as dental exams, eye exams, flu shots, races, and wellness challenges.
