Our wellness program, through Wellworks For You, allows you and your covered spouse to complete two steps to save $30 each on your medical contributions! Here are the highlights of the program.

 

• Step 1 – Complete the Physicians Results Form with your Direct Primary Care Physician or Primary Care Physician

• Step 2 – Complete 1 Preventive Exam (Annual Physical, Mammogram, OB/GYN, Colonoscopy, or Prostate Exam)
