LIFE INSURANCE
Provided at no cost to you
Four Seasons Family of Companies provides Life Insurance in the amount of one times your annual salary up to $50,000 (based on a 40-hour work week) at no cost to all regular full-time associates after 90 days of employment. Associates may name anyone as their beneficiary. Complete the Life Insurance Beneficiary Designation Form and return it to HR to designate your beneficiary(ies).
SHORT-TERM DISABILITY
Provided at no cost to you
The short-term disability program provides financial protection for you by paying a portion of your income when you are disabled. The benefit pays between 66 2/3% and 100%, based on your length of service. Click here to learn more.
VOLUNTARY LIFE INSURANCE

Four Seasons offers regular full-time associates the opportunity to purchase Voluntary Life Insurance through New York Life. During Open Enrollment, there is a guaranteed issue of up to $100,000 for associates. You can elect life insurance in units of $10,000. The maximum for any associate is the lesser of 5 times your annual salary or $500,000.
Life insurance is also available for spouses and dependents. Spouse Life Insurance can be elected at 50% of your Voluntary Life Insurance coverage up to $25,000. Dependent Life Insurance can be elected in $2,000 increments for a maximum of $10,000.

VOLUNTARY LONG-TERM DISABILITY

Regular full-time associates have the opportunity to purchase Long-Term Disability Insurance after 90 days of continuous active service.
This voluntary benefit may provide you with up to 60% of your salary if you are unable to return to any type of work after 26 weeks of company-provided short-term disability. Rates are based on your age and your salary. During Open Enrollment, there is a guaranteed issue if electing Voluntary Long-Term Disability coverage.
VOLUNTARY ACCIDENT DISABILITY

Four Seasons offers regular full-time associates the opportunity to purchase Voluntary Accident Insurance during the annual Open Enrollment benefits election. You may elect up to $500,000 in increments of $10,000.
Accident Insurance is also available for spouses and dependents. Spouse Accident Insurance can be elected up to $250,000 in increments of $5,000. Dependent Accident Insurance can be elected up to $10,000 in increments of $2,000.
